PURPOSE OF POSITION: To assume responsibility of supervising classroom placement and program referral process. To advocate for the children by assisting child/parents/Hamilton County Job Family Services (HCJFS) staff through placement, treatment and transition states. To interpret program findings, and provide follow-up visits to the parent and/or community programs. Lead and provide necessary information to the interdisciplinary treatment team.

Coordinate and facilitate activities for families in all program components, including observing visitations between children and their parents, recording and assisting parent's involvement in the child's treatment plan and programming.
·Patient Care
Participate or assist in the development of patient-specific plan of care and/or treatment. Provide care/treatment using the plan of care. Communicate the plan of care, goals, objectives, and/or outcomes with patients/families. Manage TIP intake and referral process and monitor waitlist; provide regular contact and wait status to referral source. Coordinate enrollment process with collaborative agencies. Monitor client's interagency treatment progress; schedule and attend interagency meetings as appropriate and formulate interagency treatment plans to carry out with the child or family, including collaborative treatment plans with other community professionals. Administer and process client assessments on child and family outcomes.
Manage weekly interdisciplinary treatment team meetings with staff, director and invited providers. Schedule and attend psychiatric assessments to provide treatment progress update to physician. Facilitate client medication needs/questions and consult with physician. Work collaboratively/cooperatively with others to achieve individual patient and program goals. Develop positive working relationships with interdisciplinary and interagency team members. Share knowledge and/or provide support for team members. Contribute to and promote a positive and professional work environment/atmosphere. Liaison with Head Start and local schools regarding identification and evaluation of special needs preschoolers. Assist in orientation of new staff to the program's goals and responsibilities, as well as assist Director to maintain model fidelity and positive outcomes for TIP.

Identify and understand customer needs and expectations and meets them. Communicate any relevant information to clients/families, colleagues, and other key stakeholders.
Assume responsibility for administrative functions in director's absence. Maintain and monitor charting and billing responsibilities. Oversee daily building operations and addresses needs as they arise or requested by program staff. Support the director with budget and funding operations of the Therapeutic Interagency Program (TIP).

·Bachelor of Social Work OR Special Education OR Early Childhood Education OR in a related field
·3 years related experience

·Master in Social Work OR in Counseling OR in a related disipline AND a current LISW or LPCC
