PURPOSE OF POSITION: Demonstrate advanced level clinical practice skills in providing psychiatric psychosocial assessment and intervention services to patients and families of a Medical Center dedicated to care of infants, children, adolescents, young adults, and families. Proficiency will be evidenced by use of in depth knowledge and coordination of care.

·Compliance - Patient Services Staff
Sustain a working understanding of regulatory requirements and accreditation standards. Support leadership in Compliance efforts to meet and sustain the regulatory compliance efforts of the department and hospital. Be mindful and judicious in monitoring safety and departmental policies and procedures. Ensure all individual certifications and Safety College requirements are fulfilled in a timely basis. Work with department managers, nurse council and magnet representatives to identify and share safety best practices. Be mindful of CCHMCs processes and compliance with CMS standards and freely communicate safety and compliance concerns to leadership team members.
·Interprofessional Practice:
Demonstrates consistent integration of the Interprofessional Practice Model (IPM) in all aspects of practice.
·Patient Care - Age Specific and Culturally Diverse
Consistently integrates age specific and culturally diverse concepts into patient care, taking into consideration both the patient's chronological age and developmental functioning.
·Psychosocial Assessment
Conduct a comprehensive psychosocial assessment of the patient/family's health care needs to identify the strengths, challenges, opportunities. Demonstrates broad knowledge of abuse, neglect, and other safety risk factors. Skillfully interviews patients & families to ensure an accurate history and to enhance the family's cooperation with evaluation and intervention process. Provides anticipatory guidance. Report suspicions of abuse and neglect to legally mandated authorities for investigations. Utilize the psychosocial assessment to establish treatment intervention to be included in comprehensive and effective plan of care.
·Department Representative
Assumes a responsible role on a committee and/or work group effecting the department, hospital, and/or community.
·Resource Management
Partner with the patient/family/caregivers in obtaining financial assistance, community resources, or specialized equipment. Maintains fluent knowledge of internal and external resources and individualizes recommendations based on patient and family needs. Provide real time accurate information to patient/family/caregiver for resources for which they are eligible. Provide condition-specific and related medical, financial, educational, and social supportive resource information.
·Psychosocial Interventions
Provide psychosocial services as identified in the patient's comprehensive plan of care. Creates opportunities for and provides supportive counseling with the goal of improving emotional coping and adherence to the treatment plan. Initiates collaboration with the referral source and appropriate members of the health care team in a timely and effective manner. Uses advanced knowledge and skill to educate the patient/family/caregiver and members of the health care team about treatment options. Skillfully provides self-management support to high risk/complex patients/families to increase their skills and confidence to effectively manage their chronic care conditions at home. Identifies needs, develops programs, and evaluates outcomes for special populations, as needed. Skillfully provides anticipatory guidance and planning that encourages partnership of patient/families/caregiver in reducing or eliminating psychosocial barriers to discharge. Initiate and facilitate family centered care team meetings. Anticipates points of crisis and intervenes proactively with patients and families to maintain equilibrium.
·Interdisciplinary Collaboration
Working for system improvement, promoting patient/family/caregiver well-being. Helps families become effective consumers of the health care system using knowledge of hospital, division/specialty, and/or community systems Initiates liaison role between the patient/family/caregiver within the medical team and outside agencies. Initiates role of mediator as needed with in the medical team on behalf of the patient/family/caregiver. Empower the patient/family/caregiver to problem-solve by exploring options of care when available and alternative plans, when necessary, to achieve desired outcomes. Initiates effective and efficient communication and coordination between members of the health care team while involving the patient/family/caregiver in the decision making process in order to minimize fragmentation of services.
·Continuity of Care
Partners with the patient, family, and healthcare team in the safe transition of care to the next, most appropriate level by initiating collaboration with referral sources, maintaining fluent knowledge of resources, and helping families become effective consumers of the healthcare system.
Utilizes clinical skill and knowledge of treatment options to identify those interventions that will be most successful in promoting positive outcomes, and alters interventions based on results.
Evaluates efficacy, appropriateness, and efficiency of patient and family outcomes in relation to expected outcomes through accurate and thorough identification of needs; also develops programs and evaluates outcomes for special populations, as needed.
·Professional Development
Initiates opportunities for professional growth. Educates healthcare professionals and or/ students on the psychosocial aspects of patient care and enhances the health care team's understanding of social work/clinical counselor contributions to their practice. Maintain current in the field through continuing education, literature and seminars and makes a substantial contribution to the department and/or specialty division. Models behaviors in own practice which promote a positive work environment. Actively participates in staff meetings, supervisory conferences and in-service programs to develop professional skills.

Maintain patient records and statistics on patient contact according to division guidelines.

·Master's Degree in clinical counseling program AND current license of LPCC (Licensed Professional Clinical Counselor) OR LPC (Licensed Professional Counselor) with the State of Ohio Counselor, Social Work, and Marriage & Family Board required. LPCs must be eligible for the LPCC within 6 months of the hire date. OR
·Master's degree in Social work AND LISW license with the State of Ohio Counselor and Social Work Board.
·Current License to Practice in State
·Three years of professional experience
·Promotion to this level occurs only through the Clinical Advancement Program
