PURPOSE OF POSITION: Plan and lead the design and implementation of the processes, programs and special projects to achieve the goal of the Patient Centered Medical Home (PCMH) program, which is to transform health care quality through measurement, transparency and accountability. Lead the initiatives and develop PCMH coaching offering to facilitate the successful adoption and implementation of PCMH transformation leading toward practice certification. Provide clinical coaching on coordinating patient care, and achieving quality outcomes. Provide expertise and consultative services to help practices develop customized clinical workflows for the medical home, and collaborate with physicians and practices to develop practice specific strategies and processes for PCMH transformation.

Responsibilities
·Leadership
Work with the Medical Director to determine appropriate consultative resources, pricing requirements & timelines. Oversee the PCMH coaching staff & work processes. Execute scope of program & association of projects in collaboration with leadership & stakeholders; design work plans & establish deliverables that support PCMH goals. Develop & deliver reports, proposals, requirements & presentations. Cultivate relationships vital to project success. Integrate mentoring practices on how to change clinical work flow with consistent processes for documentation & reporting. Identify sources & impacts of resistance to change, & work closely with leadership to plan, implement & evaluate change management strategies. Design, develop, evaluate & support the implementation of team-driven performance improvement initiatives. Lead near & long-term strategic project planning & direction, including the planning, direction, control, & allocation & utilization of resources within the department. Provide advice & guidance and/or render decisions on specific problems. Ensure projects improve efficiencies & effectiveness & achieve objectives; review & analyze existing processes & procedures to determine interrelationships, redundancies, adequacy, simplifications, & improvements required, & take necessary corrective action.
·Project Management
Create, execute, evaluate and update work plans to meet changing needs and requirements of PCMH recognition levels 1-3. Determine resource and/or personnel needs for each project. Identify and work to resolve problems regarding data collection, analysis and reporting, and conduct research and collect data to lead continual project development. Conceptualize the meaning of reported data and translates this knowledge for stakeholders to promote the meaningful use of data to assess patient care. Design communication tools such as webinars, workshops, and other initiatives to deliver technical assistance on PCMH topics for the practices in the program. Research PCMH and QI topics and contribute to the development and maintenance of templates, tools, and other materials and training. Track, update and display department and project data. Oversee and evaluate action plans and products to collect, consolidate, and analyze project information and requests, obtains, and collates necessary data from various project leads and/or other appropriate sources
·Coaching
Serve as a liaison with program stakeholders about progress, manage project expectations and priorities; and continuously incorporate appropriate changes. Provide specific clinical coaching and support related to practice transformation and for example, address practice needs related to enhancing access and continuity, identify & manage populations, plan and manage care, provide self-care support & community resources, track & coordinate care, measure and improve performance. Coach, mentor, motivate and oversee project team members and contractors, ensuring their individual accountability for assigned work. Cooperate with other departments, stakeholders, and consultants to plan and deliver various trainings related to performance integration and assigned projects. Provide detailed orientation to key personnel on duties and interactions within the organization pertaining to PCMH.

·Knowledge of Field:
Proven ability to coordinate patient care and achieve quality outcomes such as improved patient experience, improved clinician engagement, reduced cost of care and improved quality of care through implementation of evidence based practice. Capable of utilizing research techniques in application to nursing practice. PCMH transformation, and serving as a resource for educational opportunities in these areas. Possess motivation to promote continuing education, practice transformation and family-centered care. Ability to coordinate patient care and achieve quality outcomes. Knowledge of PDSA and continuous improvement methodology in the clinical setting

·Preferred:
Excellent research and writing skills. Strong analytical, organizational, project management and time-management skills. Proficient with MS Office Suite. Working knowledge of clinical processes or ability to learn EMRs, healthcare quality and data management systems.

Required:
·Bachelor's Degree OR equivalent experience
·RN Diploma
·Obtain NCQA PCMH Content Expert Certification within 6 months after hire
·7 years related experience
·Experience in development and implementation of PCMH models AND knowledge of NCQA Recognition Program

Preferred:
·Master's Degree
·Healthcare experience
·Experience with practice transformation and change management
