·Compliance - Patient Services Staff
Sustain a working understanding of regulatory requirements and accreditation standards. Support leadership in Compliance efforts to meet and sustain the regulatory compliance efforts of the department and hospital. Be mindful and judicious in monitoring safety and departmental policies and procedures. Ensure all individual certifications and Safety College requirements are fulfilled in a timely basis. Work with department managers, nurse council and magnet representatives to identify and share safety best practices. Be mindful of CCHMCs processes and compliance with CMS standards and freely communicate safety and compliance concerns to leadership team members.
Demonstrates consistent integration of the Interprofessional Practice Model (IPM) in all aspects of practice.

Consistently integrates age specific and culturally diverse concepts into patient care, taking into consideration both the patient's chronological age and developmental functioning.
Conduct a comprehensive psychosocial assessment of the patient/family's health care needs to identify the strengths, challenges and opportunities. Evaluate patient/family/ caregiver safety concerns. Intervene appropriately to reduce risk. Report suspicions of abuse and neglect to legally mandated authorities for investigations. Utilize the psychosocial assessment to establish treatment intervention to be included in comprehensive plan of care.
Serve on committees and specialized task groups effecting the division, hospital and or community
Partner with the patient/family/caregivers in obtaining financial assistance, community resources, or specialized equipment. Maintain fluent knowledge of internal and external resources. Provide real time accurate information to patient/family/caregiver for resources for which they are eligible. Provide condition-specific and related medical, financial, educational, and social supportive resource information.
Provide psychosocial services as identified in the patient's comprehensive plan of care. Provides supportive counseling with the goal of improving emotional coping and adherence to the treatment plan. Collaborate with referral sources and health care team members. Education of the patient/family/caregiver and members of the health care team about treatment options. Provide self-management support to high risk/complex patients/families to increase their skills and confidence to manage their chronic care conditions at home. Provide groups and programs for special populations as needed. Provide anticipatory guidance and planning to reduce or eliminate psychosocial barriers to discharge. Coordinate and facilitate family centered care team meetings. Provide crisis assessment and interventions to patients/families/caregivers with the goal of restoring equilibrium.
Working for system improvement, promoting patient/family/caregiver well-being. Identify and advocate for the needs of the patient/families/caregivers within the healthcare system and community. Act as a liaison between the patient/family/caregiver within the medical team and outside agencies. Act as the mediator as needed with in the medical team on behalf of the patient/family/caregiver. Empower the patient/family/caregiver to problem-solve by exploring options of care when available and alternative plans, when necessary, to achieve desired outcomes. Facilitate communication and coordination between members of the health care team while involving the patient/family/caregiver in the decision making process in order to minimize fragmentation of services.

Partner with the patient, family, healthcare team in the safe transition of care to the next, most appropriate level.
Evaluates effectiveness of social work interventions, and alter interventions based on results.
Evaluate efficacy, appropriateness, efficiency patient and family outcomes in relation to expected outcomes.
Participates in professional growth opportunities. Educates healthcare professionals and or/students on the psychosocial aspects of patient care and the roles and functions of social work. Maintain current in the field through continuing education, literature and seminars and implement that knowledge into the specialty or division. Promotes a positive work environment. Attends staff meetings, supervisory conferences and in-service programs to develop professional skills
