PURPOSE OF POSITION: To provide technical assistance to clinical practices in improvement of care delivered to patients such that it is higher quality, evidence based, efficient, coordinated and patient-centered. PICs mentor clinicians in fundamentally changing the way they deliver care through integration of quality and process improvement activities, culture change, use of established practice transformation tools and developed learning sessions as well as patient and family engagement. PICs champion the building of skills necessary for a clinical practice to achieve and sustain ongoing change and quality improvement efforts. As requested by clinical practices the PIC will lead initiatives and coach clinical practices in developing patient-centered transformation strategies to facilitate successful application submission and NCQA recognition .
Execute scope of program and association of projects in collaboration leadership and stakeholders; design work plans and establishes deliverables that support organizational goals for practice transformation. Develop and deliver reports, proposals, requirements documentation and presentations. Cultivate professional relationships vital to the success of the projects. Integrate, through collaboration with internal stakeholders, mentoring practices on how to change clinical work flow with consistent processes for documentation and data reporting. Identify sources and impacts of resistance to change, and work closely with leadership to plan, implement and evaluate change management strategies. Design, develop, evaluate and support the implementation of team-driven performance improvement initiatives. Provide advice and guidance and/or renders decisions on specific problems. Ensure projects improve efficiencies and effectiveness and achieve business and clinical objectives; review and analyze existing processes and procedures to determine interrelationships, redundancies, adequacy, possible simplifications, and improvements required, and take necessary corrective action
Create, execute, evaluate and update work plans to meet changing needs and requirements of NCQA PCMH recognition levels 1-3. Determine resource and/or personnel needs for each project. Identify and work to resolve problems regarding data collection, analysis and reporting, and conduct research and collect data to lead continual project development. Conceptualizes/ the meaning of reported data and translate this knowledge for stakeholders to promote the meaningful use of data to assess patient care. Design communication tools such as webinars, workshops, and other initiatives to deliver technical assistance on PCMH topics for the practices in the program. Research Practice Transformation, Care Coordination, Referral Tracking, Care Transitions, Patient-Centered Medical Homeness, Team Based Care and QI topics and contribute to the development and maintenance of templates, tools, and other materials and training. Track, update and display department and project data. Oversee and evaluate action plans and products to collect, consolidate, and analyze project information and request, obtain, and collate necessary data from various project leads and/or other appropriate sources.
Serve as a liaison with program stakeholders about progress, manage project expectations and priorities; and continuously incorporate appropriate changes. Coach, mentor, motivate and oversee project team members and contractors, ensuring their individual accountability for assigned work. Cooperate with other departments, stakeholders, and consultants to plan and deliver various trainings related to performance integration and assigned projects. Provide detailed orientation to key personnel on duties and interactions within the organization pertaining to PCMH.

·Bachelor's Degree OR equivalent experience
·5 years related experience
·Obtain NCQA PCMH Content Expert Certification within 6 months after hire
·Experience in development and implementation of PCMH models AND knowledge of NCQA Recognition Program Preferred:
