The Outreach Specialist will work with the Community & Population Health Sr. Director and others to identify and align evidence-based team building, quality improvement methods, leadership development, engagement and activation strategies with outcomes of the All Children Thrive Learning Network. The goals of these new strategies will be to
  • Bring families, residents, public and private organization partners together across sectors to collaborate using quality improvement and family centered designs toward improved health outcomes.
  • Develop and implement family centered training focused on improving systems of health, education and wellness
·Community Partnerships
Initiate and maintain a positive relationship with parents, medical providers, community service and education providers. Gain a comprehensive knowledge of community resources and agencies serving families and children and the skills to forge working relationships with them. May represent the All Children Thrive network on community boards and/or committees of organizations that support the program objectives, where appropriate. Work closely with community to ensure that they have the necessary tools and support to ensure successful implementation of the program. Set the stage for constructive relationships by being accessible, approachable, and interpersonally engaged, developing and sustaining meaningful and productive bonds with others based on mutual respect
Demonstrate ability to coordinate daily activities of a team. Ability to function independently and as a part of a team. Act as a resource for problem solving. Serve as liaison to appropriate collaborative partners and CCHMC departments for resolution of issues as needed. Develop and maintain cooperative relationships with all Cincinnati Children's departments and outside agencies with which the department interacts.
·Program Planning-Management and Execution
Participate fully in the planning process identifying areas of need and focus and translating portions of the program plan in to well defined projects and programs w/ timelines and deliverables. Plan and execute projects in accordance with agreed upon project timelines. Collaborate in an effective and timely manner with internal and external contacts to achieve program goals. Support and contribute to the program planning and reporting process of the organization. Track program metrics and goals and prepares internal reports. Take an organized approach to work and planning ahead; identifying objectives aligned with business strategies, structuring work, and estimating necessary time and resources; anticipating and adjusting for potential obstacles and problems; monitoring work progress.
Develop and deliver presentations to internal and external partners that promote and detail the program priorities and strategies. Perform public speaking and train diverse groups of professionals. Write proposals and secure project funding.

Ability to develop materials for presentations to a variety of audiences both internal and external.
Capable of relating to diverse age and demographic backgrounds.
Ability to manage multiple projects simultaneously, work both independently and in a team environment, and work efficiently and creatively to meet deadlines.
Leadership and strategic planning skills and the ability to implement projects successfully.
Computer literate and working knowledge of software word process, spreadsheet and desktop publishing applications.

Prioritization and time-management skills sufficient to meet deadlines in a fast-paced environment.

Knowledge of key CCHMC communities, including those we serve and those where we have locations. Knowledge of the social determinants of health. Ability to work cooperatively and successfully with community organizations. Skills in building positive relationships with senior leaders, managers, employees, community leaders and neighbors. Capable of managing relationships with external vendors

·Bachelor's degree in related discipline
·3 years related experience

·Social Services, Healthcare or health-related experience preferred
·Experience with medical home providers, community collaboratives and social services
