PURPOSE OF POSITION: To function as a professional nurse and to apply in depth knowledge of diabetes in the care of infant, children, and adolescent patients and their families. To provide clinical management and diabetes education for hospitalized pediatric patients, and assistance with clinical administrative issues within the diabetes practice. Provide education and consultation on the management and/or prevention of diabetes. To rapidly recognize and anticipate subtle changes based on past experiences; to coordinate and communicate with appropriate resources as needed. To serve as a department-based role model and resource for policies, procedures and nursing care. To assist others in setting nursing care and department priorities, keeping in mind the education of self and others while maintaining a cohesive environment.

Responsibilities
·Interprofessional Practice:
Demonstrates consistent integration of the Interprofessional Practice Model (IPM) in all aspects of practice.
·Clinical Practice
Uses patient age-appropriate knowledge and skills to effectively manage diabetes diagnosis. Provide specialized expertise to patient and families in the prevention, regulation and management of diabetes. Provide clinical nursing care to patients with an insulin pump. Collects patient data to monitor health status and proactively manage the patient's condition. Considers patient's and family's cultural preferences, health beliefs and preferred learning styles. Includes assessment, planning implementation, outcome identification and evaluation of intervention. Documents and collaborates in the plan of care. Responsible for helping to maximize metabolic control by providing diabetes self-management education including: assessment, goal setting planning, implementation, monitoring, evaluation and collaboration between the patient and the diabetes team. Aids in facilitating management for the child in the school setting.
·Care Integration
Works as an effective member of a multi-disciplinary pediatric diabetes care team (MDs, nurse practitioners, nurses, dietitians, and social worker), and working with children with diabetes. Maximize the use of interdisciplinary team resources to enable chronic disease management for patients with diabetes. The incumbent will focus on diabetes care, and preventative treatment. Provide direct patient management, education, and discharge planning to children/families with new onset and ongoing management of the condition post-diagnosis in the outpatient setting. Works collaboratively with the treating provider to manage and monitor treatment and the response to therapy. Provides assistance to the patient and family in obtaining necessary prescriptions for supplies and medication. Partners with coworkers to provide valuable input on projects and takes ownership of project(s) as assigned
·Patient and Family Education
Provides pre-diabetes and diabetes instruction in group and individual sessions. Obtains individual patient assessment. Identifies patient/family problems or learning barriers including physical, cognitive, environmental, language/cultural, financial, psychosocial and spiritual and provides appropriate referral as necessary. Establishes individualized learning goals, objectives and educational plan in collaboration with the patient and the patient's caregiver(s).Evaluates patient upon completion of educational plan and provides remediation/plan modification as indicated and in consultation with diabetes team members. Develop educational materials on diabetes self- management based on needs of families or requests by staff in accordance with Clinical Content Committee guidelines, including materials on diabetes self- management, insulin adjustment, use of insulin pumps, and effective educational strategies for families with diabetes on patients from diverse ethnicities for both type 1 and type 2 Diabetes Mellitus. Stress the importance of properly managing diabetes to lower the risk of complications and reduce the chances of hospitalization.
·In-Service Staff Education
Demonstrates willingness to actively participate and accept responsibility for special projects such as speaker requests, and diabetes family programs/events. Participates in healthcare professional education programs. Provide ongoing education and consultation to nursing and other ambulatory staff, supports and provides physician education, and supports clinicians in hospital. Orients and educates new staff. Shares knowledge of pediatric diabetes principles, policies and procedures as applicable. Educate and train health care professionals in pediatric diabetes. Develops appropriate training materials.
·Out-reach
Participates in department projects, including ADA Program Recognition and inpatient initiatives. Participates in community activities as requested, including education endeavors, for patients and families with diabetes including but not limited to: presentation of educational lectures to children, parents, healthcare professionals and the community in general; diabetes family programs and events.

Evaluates effectiveness of educational services provided by measuring attainment of learning objectives.Conducts a follow-up assessment upon completion of program services, using outcome measures. Documents assessment data, educational plan, educational services provided and evaluation results in each participant's educational records

Complies with the American Diabetes Association Education Recognition Program requirements for continuing education until CDE credential is obtained. Seeks professional growth opportunities to advance care and knowledge related to diabetes.
·Regulatory Compliance
Documents all educational assessment, planning, intervention, behavioral goal setting, and outcomes accordingly in the medical record accurately and within the expected time frame for charge entry, monthly statistics and ADA recognition. Complies with the applicable standards for diabetes self-management education and self-management support set forth by the American Diabetes Association and with the Diabetes Education Policies and Procedures of Cincinnati Children's Hospital Diabetes Center, and other recognition or certification programs.

Required:
·Bachelor of Science in Nursing OR Master's Degree in a related field
·Current license to practice nursing in the State of Ohio
·2 years of experience in care and management of diabetes and other related disorders
·Obtains CDE certification within 36 months of hire and maintains certification
·Obtains and keeps current pump certification(s) for patient/family education as directed

Preferred:
·Certified Diabetes Educator
·Experience in caring for newly diagnosed pediatric diabetes patients
