PURPOSE OF POSITION
To coordinate the triage and referral of patients into appropriate Behavioral Medicine and Psychological Services within a complex medical center and the Behavioral Medicine and Psychological Services community dedicated to the care of infants, adolescents and children.
I. Triage and Assessment: Triage and evaluate the patient need vs. service availability of patients and families for outpatient referrals. Advocates for the needs of the child or adolescent patient and family within the hospital and the community. Assess patient need to determine phone call urgency. If routine, assess needs and provide specific referral options. If urgent, transfer to appropriate services.
II. Scheduling Appointments: Completely and accurately schedule new appointments; utilize EPIC and BMCP processes correctly and efficiently.
III. Project Participation/Coordination: Assist in the coordination and development of special projects as requested by Director to develop and maintain efficiencies of BMCP Intake operations.
IV. Collaboration: Work closely with BMCP Clinical Leadership, ASRs and clinical providers; collaborate with Psychiatry, DDBP and other CCHMC Divisions to best assist patients and families.
V. Customer Service: Demonstrate excellent customer service on every phone call; use appropriate scripts and resources.
Bachelor's Degree in Social Work, Counseling, or related degree.
5 years related experience
Master's Degree in Social Work or related field
