SUPERVISOR'S TITLE: Clinical Manager/ Clinical Director

PURPOSE OF POSITION: To serve as an assistant to a social work team who provide services to patients and families in a pediatric healthcare setting. To support the delivery of care to patients and families at a Medical Center dedicated to the care of infants, children and adolescents. Assist members of a multidisciplinary team of social workers in their treatment and interventions for patients. Collaborate with the department leadership to facilitate the smooth operation and care delivery.

Responsibilities
·Family Centered Care
Respond promptly to referrals from a variety of sources to assist the team in providing comprehensive care to patients and families. Conduct follow up phone calls to families as assigned and assist with adherence issues. May arrange transportation and lodging for families as directed by social workers. May follow up on all reservations, verify billing charges for transportation and lodging services.
·Collaboration
Work collaboratively with internal staff and external resources to ensure services are satisfactory. Participate in interdisciplinary teams and medical center committees as appropriate. Develop relationships with local, regional and national, resources and networks.
·Foundation Requests and Support
If involved in supporting a foundation will assemble and sends all documentation for foundations, wish organizations. Coordinates and enters all foundation data into database. Provides quarterly reports to administration and foundations. Sets up foundation recognition and sending thank you cards.
·Resource Materials
Develop and maintain knowledge of appropriate internal and external (e.g. community) services and resources for patients and families. Maintain updated resource materials and contact information available for families/providers.
·Data Tracking
Maintain data on program specific metrics such as the number of referrals, outcomes, and disposition. Develop and maintain reporting to support tracking and decision making processes. Report back to referral source as to action taken/results obtained.
·Data Entry
Performs clinical data entry and validation to ensure accuracy, completeness and legibility of data including multiple questionnaires/test sets. Assists with the development and evaluation of forms. Enter process and clinical data into various databases.
·Quality Assurance
May perform quality checks to ensure accuracy of data manually entered by others. Maintains and audits data, providing status and activity reports as required. Maintains and revises lists, control records, and coding schemes necessary to process source data. Maintains master files for clinical protocol.
·Administrative Support
Assists users with requests for clinical documents. May answer phones, file paperwork, provide customer service and assist with other administrative processes.
·Patient Education/Advocacy
Provides patient/family with education, information, and referral services- may provide patient/family with relevant information about illness, insurance coverage, support and community services.
·Department Support
Assist in the coordination and development of special projects as requested by Clinical Manager. Support assigned programs, including arranging food, developing flyers, & coordinating mailings. Provide assistance to social workers for all patient and family education and support programs.

Required:
·Associate's Degree OR Equivalent combination of education and experience
·Knowledge of medical terminology

Preferred:
·Bachelor's Degree
·2 years of experience in a health care office environment
